- 8:30 AM: On Court Stretch
- 8:45 AM: Welcome, Murphy Speech, & Prayer
- 9:00-11:00 AM: Adult Clinic
- 11:00 AM-12:30 PM: Lunch, Exhibition & JPO Awards
- 12:45 PM: Kids Registration
- 1:00 PM: Session with Murphy for Kids
- 1:15-2:15 PM: Session for Kids aged 5-10
- 1:15 PM to 2:30 PM: Session for Kids aged 5 and up
All proceeds will be used to educate area students on promoting healthy relationships and distributed to local non profit mental health providers specifically directed at making mental health care more accessible and timely in our community. The JPO Foundation is a registered non-profit organization.
Release of Liability for Adult Participants
You have been invited to the JPO Charity Tennis Event on September 13, 2025.
I RELASE AND AGREE TO PROTECT AND HOLD YOU, THE FOUNDATION AND THE TWO’S ATHLETIC CLUB AND THEIR RESPECTIVE HEIRS, AGENTS, EMPLOYEES AND ASSIGNS (COLLECTIVELY, THE “OFFICERS” AND OWNERS OF THE CLUB) HARMLESS FROM AND AGAINST ANY AND ALL CLAIMS, DEMANDS, CAUSES OF ACTION AND DAMAGES, INCLUDING ATTORNEYS’ FEE, RESULTING FROM ANY ACCIDENT, INCIDENT OR OCCURRENCE ARISING OUT OF INCIDENTAL TO OR IN ANY WAY RESULTING FROM PARTICIPATING AT THE JPO CHARITY EVENT. THIS RELEASE AND INDEMNIFIACTION APPLIES DURING ANY TIME DURING THIS EVENT. I ALSO PROMISE AND AGREE THAT I, MY HEIRS, SUCCESSORS AND ASSIGNS WILL NOT MAKE ANY CLAIM OR INSTITUTE ANY SUIT OR ACTION AT LAW OR IN EQUITY AGAINST ANY ONE OR MORE OF THE OFFICERS OF THE JPO FOUNDATION AND THE OWNER OF THE TWO’S ATHLETIC CLUB BY REASON OF CONDITIONS OF THE LEASED PREMISES OR ACTIVITES OCCURING THEREON. THIS RELEASE INCLUDES MINORS IN MY CARE WHILE AT THE JPO CHARITY TENNIS EVENT.
I understand that you are relying on this Release in allowing me to enter the JPO Charity Tennis Event and participate in all the activities there and intend for you to do so. I also understand that this Release is a requirement for my entry and participation.
PHOTO AND FILM PERMISSION AND RELEASE I hereby grant permission to the JPO Foundation to use photographs and film taken of me at the JPO Charity Tennis Event related functions or from a scheduled photo shoot or videotaping/filming session on the JPO Foundation web site, JPO Foundation social media accounts, or other JPO Foundation medium, and to offer the photographs and film to other organizations that the JPO Foundation is currently in partnership with such organizations as the University of Toledo Medical Center and its departments, JED Foundation, One Love Foundation, ProMedica Teen Pep and any other organizations that promotes healthy relationships and crisis intervention for mental health. I hereby waive any right to inspect or approve the photographs, film, or any electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown. I waive any right to royalties or other compensation arising from or related to the use of the photographs and/or film. I hereby agree to release and hold harmless the JPO Foundation, from and against any claims, damages or liability arising from or related to the use of the photographs and film, including but not limited to any reuse, distortion, blurring, alteration, optical illusion or use in composite form, either intentionally or otherwise, that may occur or be produced during the production of the finished product. I am 18 years of age or the parent or legal guardian of a person under the age of 18 and I am competent to contract in my own name. I have read this release before signing below, and I fully understand the content, meaning and impact of this release. I understand that I am free to address any specific questions regarding this release by submitting those questions in writing prior to signing, and I agree that my failure to do so will be interpreted as a free and knowledgeable acceptance of the terms of this release.
